Mailing Documentation or Institutional Endorsements to DPI Separately from Original Application
If any license application documentation will be mailed separately from the license application and fee originally sent to DPI, or if a copy of the application has been sent to an institution of higher education for endorsement of the educator preparation program, the instructions below apply.
- Do include the applicant's DPI educator file number on each document submitted separately. Your DPI Educator File Number is assigned the first time that we enter an applicant's information into our system and can be obtained from the Educator License Lookup. Search by entering your name and then make a note of the six-digit file number that appears on your record.
- Do not send a copy of the original application form with the document.
- An item on a document may be circled or underlined to bring it to our attention. However, highlighters must not be used as they make information unreadable after documents are optically scanned.
- The applicant should keep a copy of each document submitted for personal records.
- To ensure that institutional endorsements (forwarded to DPI by a college or university) are mailed to the correct address, applicants should provide the mailing address or a copy of this page to the college when sending forms to them for endorsement.
