Request Fingerprint Cards from DPI

Request Fingerprint Cards by Email

In your email, you must include:

• First and Last Name
• Complete mailing address: Street, City, State, Zip Code
• Six Digit Educator File Number (If you have previously held a Wisconsin educator license.)
  Your DPI Educator File Number can be obtained from the Educator License Lookup. Search by entering your name and then make a note of the six-digit file number that appears on your record.

E-mail Karen Bomkamp. (Please allow 1 week to receive the fingerprint cards and instructions.)

Request Fingerprint Cards by Phone

• Phone the department at 1-800-266-1027.
• If you do not reach a staff person, Choose Option 1 from the Voice Mail Menu
• Leave your message requesting fingerprint cards along with your complete name, mailing address, and Educator File Number.
