Request Fingerprint Cards by Email

In your email, you must include:

• First and Last Name
• Complete mailing address: Street, City, State, Zip Code

E-mail DPI Fingerprint Cards. (Please allow 1 week to receive the fingerprint cards and instructions.)

Request Fingerprint Cards by Phone

• Phone the department at 1-800-266-1027.
• If you do not reach a staff person, Choose Option 1 from the Voice Mail Menu
• Leave your message requesting fingerprint cards along with your complete name and complete mailing address.
