Educator License Application Forms

Install or Update Acrobat Reader

For best results, we recommend downloading the newest version of Acrobat Reader from Adobe's website. With versions of Acrobat Reader prior to 7.0, you may experience problems downloading the application forms and/or using the fill-in fields.

NOTE: If you are using Acrobat Reader version 6.0 or earlier, you will be able to save fill-in forms containing entered information. When you "save" a fill-in pdf form in those versions, all entered information is lost. You must print a paper copy of your application before closing/saving the form!

If you do not wish to use fill-in pdf forms, you may request an application from Educator Licensing. Please specify the PI application form number in your request.
