Educator License Application Forms

Printing Standards for Applications Mailed to DPI

Printed application forms will be optically scanned at DPI and must be clear, legible, and of good quality. Poorly printed and non-standard size application forms may be returned to you, requiring resubmission of a legible printed copy and causing delay in processing your application for licensure. If you are completing the form by hand, use either black or blue ink only.

Use Standard 8 1/2" X 11" White Paper with Black print, Portrait format, One-sided only:  DO NOT send applications printed "back to back."   During processing, sections of your application are separated and distributed to various units of the Teacher Licensing Team. Submitting only one-sided forms will ensure that each unit has all of the pages and information needed to process your application.

If using an InkJet printer that requires you to "Shrink to Fit" to print the application form, you must enlarge a copy back to normal size before mailing it to DPI.  Important: Print all pages of the application including the Payment Section (page 1) and the Conduct and Competency Review.

Use a Good Quality Printer and Black Ink:   For best results, use a Laser Printer.   Do not use the "economy mode" of a printer which may produce a copy which is too light for optical scanning.   We do not accept forms printed on a dot-matrix printer since they will not be legible in our scanning system.

Suggestions for Specific Printers

  • Canon BJC 4100 - PDF files may not print properly in Photographic mode. Instead, use the Graphics and Text or Automatic modes.
  • Canon LBP-81V - For best results, use Bitmap mode.
  • HP DeskJet Printers - If available, use Raster Graphics rather than Vector Graphics mode.
  • HP DeskJet 500C - This printer has an unprintable area near the top of the page. Use the shrink-to-fit option in Acrobat Reader's Print dialogue box, then photocopy and enlarge a copy back to normal size.
  • HP LaserJet Printers - Use the printer driver's TrueType font option to download TrueType fonts as bitmaps rather than outlines.
  • HP LaserJet 5M - Use the standard HP LaserJet 5M printer driver rather than the enhanced driver that came with the printer.

Other Printing Problems:   Some printing problems are due to using older versions of Acrobat Reader. See Check Reader Version. Also refer to your printer documentation or contact Customer Support of the printing manufacturer. You may need to adjust printer settings or update the print driver. Adobe's Customer Support may be able to provide assistance.

DPI cannot provide technical assistance with printer hardware and software. If you experience problems printing the application and would prefer to have a paper application mailed to you, please notify Educator Licensing to request the application form you need by name and PI number.
