Mandatory School District Reporting of Educator Misconduct


Under certain circumstances outlined in section 115.31(3) of Wisconsin Statutes, school district administrators are required to report to the state superintendent the name of a person employed by the educational agency.

To assist districts in fulfilling the requirement, the PI-1620 License Review Referral form MS Word document is provided as the reporting mechanism.

The PI-1620 License Review Referral form should be completed and mailed to:
