Your license is still valid under your previous name. However, if you do want a new license certificate reflecting the new name, you must complete the PI-1602-DUP form to request an updated duplicate license. There is a $50 fee for a duplicate license. When you next renew your license, your name and/or address can be changed on our records at that time and the license certificate issued then will reflect the current information.
