Welcome to the opening page for the online Parent Involvement survey.

Please have ready the letter that your school district sent to you. It has important information that you will need to enter in the survey.

After you click on one of the links below, you will come to a page with two boxes. Enter the parent Username into the Username textbox and the parent Password into the Password textbox when prompted.

If the child named in your letter was:

• six years old or more before September 1, 2011, please enter the 6-21 survey.
• under six years old before September 1, 2011, please enter the 3-5 survey.
