The print versions of the open enrollment application forms (English and Spanish) are available for download, as 
follows:

Spanish version - http://dpi.wi.gov/forms/pdf/pod9410-spanish.pdf 
 
Although print versions are made available, we still strongly encourage parents to submit their applications on-line.  
 
The forms currently on the web site do not yet contain the Virtual Charter School list, as we are still finalizing it.  If a 
parent receives a form without the Virtual Charter School list, the parent can still apply to the virtual charter by 
indicating the name of the school in the “specific school or program” space on the application.   I’ll send an email 
when the complete form, including the Virtual Charter School list, is on the web site.
