The paper OE application form is located at http://dpi.wi.gov/forms/pdf/pod9410.pdf.   You can print as many as you 
need.   Of course, we strongly encourage parents to apply on-line.  
 
Please note that the virtual charter school supplement page is out-of-date.   We have been notified of some 
additional virtual charter schools and changes in others. There may be more before the beginning of the application 
period.  We have sent the information we have so far to our forms team for an update, but it simply takes more time 
to update a paper form than an on-line version.  
 
If you are aware that a parent wishes to apply to a virtual charter school, please encourage them to do so on-line, so 
that they will have an updated list of all virtual charter schools.   If they cannot apply on-line, please have them 
contact you or the DPI at the beginning of the application period so we can provide the most up-to-date virtual 
charter school information.
