Important: The following information applies only to schools and libraries that will be purchasing cell phone services from the new state Department of
Administration's (DOA) cell phone contract which will start July1, 2007. Applicants always have the option of submitting their own 470 and working directly with the cell phone provider of their choice, outside of the state contract.

Background
The current state Department of Administration (DOA) cell phone contract expires on June 30, 2007. DOA will be bidding for a new cell contract to start July 1, 2007. However, this new or "replacement" contract will not be signed before the February 7, Form 471 filing deadline. Thus, to enable schools and libraries to purchase off the replacement cell contract, you must follow the SLD guidance on State Replacement Contracts, Scenario A. The information below follows the SLD guidance in this process. Under Scenario A, DOA must file the Form 470, which it has already done.
The funding commitment made under the new cell contract will be limited to the charges you are being billed under the current contract. DOA does not expect increases in costs in the new contract, but in the small chance there is a slight increase, the increase can be reflected in your 471 filed in subsequent years.
State Replacement Contract and the 471 Block 5
Complete the information requested on the 471, blocks 1-4, in the usual manner.
Following the SLD's State Replacement Contract process, below is the information to enter
in the 471 Block 5. (See also the Block 5 screen image below.)

State Replacement Contract, Follow-up Actions
The actions listed below follow the SLD process for State Replacement Contracts.
After DOA signs the state replacement contract for cell services, you must manually mark-up your Funding Commitment Decision Letter (FCDL) making various changes as directed below. While this may appear to be somewhat complicated, it is really a straight-forward process. DPI will provide the documentation requested below after DOA signs the contract, which is expected to be in May or June 2007.
- On the FCDL you will request a SPIN change by replacing the service provider designation "State Replacement Contract" (from #14 above) with the name of the actual vendor who has been awarded the replacement cell contract. On your FCDL you must also make the changes below, if needed, to indicate:
- the appropriate Funding Request Number (FRN)
- the actual service provider name (attach appropriate page of the contract)
- the actual service provider's SPIN
- the DOA's Form 470 number which was filed for the replacement contract (i.e., 153990000609552)
- the contract award date (attach appropriate page of the contract)
- the service start date
- the service end date
- the replacement contract number
- the actual expiration date of the replacement contract (attach appropriate page of the contract)
- the effective date of the replacement contract.
- You must sign and date the marked-up page(s) of the FCDL. Return it by registered or express delivery to:

- You will receive notice from the SLD when the SPIN change has been made.
- Once you receive the notice, you then submit a Form 486 using the replacement contract information.
- In subsequent program years when you file a Form 471, you will reference
the DOA's Form 470 application number (153990000609552) as the establishing Form 470 for this replacement contract for cell services.
