Important: The following
information applies only to schools
and libraries that will be using the state Department of Administration
(DOA) contract to receive discounts on (1) cell (voice) phone services, and/or (2)
data or Internet services received via handheld devices. Applicants always have the option of
selecting the provider of their choice, outside of the state contract. If you
will be purchasing services from the DOA contract note the following:
- To receive discounts on only voice cell phone services applicants can file their own 470 or they have the option of using the state Department of Administration's 470. In either case the applicant must file their own Form 471.
- To receive discounts on data or Internet services (e.g., email, Web)
received via any handheld device (cell phone, BlackBerry, etc.)
applicants must file both a Form 470 and Form 471.

CONTENTS
1) Background TOP
The Department of Administration currently has a statewide contract for wireless voice & data services. The contract became effective on December 1, 2007. (See the DOA cell contract Website for details.) This
contract can be used to apply for E-rate discounts for 2009.
Under SLD E-rate rules the DOA contract is regarded as a State Master Contract (SMC).
Under the SMC process, DOA submitted a Form 470 for voice cell services
(but not for data/Internet services received via handheld devices) prior to starting
the bid process earlier in 2007. The three vendors listed below are providing
services under the contract. See
the map of state regions for
specific coverage and use the links below to review information each provider has on its various service plans, costs, etc. Some services and costs that apply to state government agencies differ from those offered to authorized users. (Schools and libraries are "authorized users.")

If you have more specific questions related to a particular service from a provider, and if you already have an account representative, then contact that person. Otherwise, vendor representatives are listed in the above basic information links. Note also that Cellular One, the original provider on the DOA cell contract, has been acquired by AT&T Mobility. Applicants should use AT&T Mobility as the provider. Some DOA documents still refer to Cellular One.

3) Handheld Devices and Internet Access

If you want to use the DOA contract to receive discounts on data/Internet services
(e.g., email, Web) received via any type of handheld device (cell phone, BlackBerry, etc.),
you must file both a Form 470 and Form 471. (The DOA 470 was filed only for voice cell services, not data/Internet access.) In completing the 470,
note the following:
- In Block 2, #7, check a, b and both boxes under b.
- In Block 2, be certain to check #9, Internet Access.
- If you want to receive discounts on both traditional (voice) cell services and data/Internet Service under the DOA contract, you may want to file your own Form 470 for both services. In this situation, to request funding for voice cell services on your 470, be certain to also check #8, Telecommunication Services, in addition to #9, Internet Access.
- After waiting the required 28 days, if no vendor has responded to your 470 with a better offer than provided via the DOA contract, then file your 471. For Item #12 on the 471, enter the number of the Form 470 that you filed.
4) Companies Doing Business As U.S. Cellular and Inactive SPINs

There are many companies doing business as (dba) U.S. Cellular. The following is a list of these companies that DPI is aware of which will honor the DOA contract with U.S. Cellular. If you are getting services from one of these other companies, then you can list it on your 471, but use the U.S. Cellular contract # of 15-91575-701. However, some of these companies are still listed in the SLD database but their SPINs are not active. If the company is listed as inactive, use U.S. Cellular as the provider and use the U.S. Cellular SPIN of 143011191. The list of inactive SPINs has been provided to DPI by U.S. Cellular staff.
