Electronic budget format for ALL Sponsoring Organizations of Affiliated Sites for the 2010 program year. This form should be downloaded to your computer, completed in full, saved, and sent as an attachment in an E-Mail to: cari.muggenburg@dpi.wi.gov. Completion and submission of this budget is required for the FFY 2010 application.

At Risk After School Hours Care Sites

• Documentation of Area Eligibility. Each site must be located in an area served by a school in which at least 50 percent of the enrolled children are certified eligible for free and reduced price meals.
