Supplemental Educational Services (SES) Provider ReportingIn order to remain on the list of organizations approved to provide SES tutoring in Wisconsin, all SES providers must submit all required reports. These reports will be required of all SES providers, whether or not they actually enroll and serve students in a particular year. Approved SES providers may also be required to complete updates of their service or program data. All providers will also have an opportunity to update program information annually as needed. SES Provider Year-End Report: The year-end report must be completed on-line. To access the year-end report providers must have the password that was created to complete the initial application. Additional information and instructions are included in the on-line report which can be found at http://www2.dpi.state.wi.us/ESEA_SES/MainMenu.asp The deadline for submission of the SES Provider Year-End Program Report is 3:00 p.m. on August 1. Assurances and Debarment Forms: Approved SES providers in Wisconsin are annually required to submit a new Assurances and Certification of Debarment form. Program Updates: Approved SES providers may be required to make changes and updates to the program service data or educational program plans on file with the DPI. Failure to submit required SES reports may result in a provider being removed from the list of organizations authorized to contract with districts in order to offer services in Wisconsin.
