Supplemental Educational Services (SES) Provider Application Information


The application for new providers to join the Supplemental Educational Services Program must be completed on-line. Detailed instructions are provided in the online environment. Each applicant will have to create a unique password to begin the application process. The application will require information about the type of services the organization intends to provide and documentation of the applicant's ability to provide an educational program that will meet the needs of eligible students and their families. Applicants must also sign and submit an Assurance form and provide evidence of financial soundness.

Application Form for New Providers: The SES application window is January-April. The online application can be found at: http://www2.dpi.state.wi.us/ESEA_SES/MainMenu.asp

Assurances and Certification of Debarment Forms: SES provider applicants in Wisconsin are required to submit both the Assurances and Certification of Debarment forms. Paper copies with an original signature must be submitted to the DPI by the deadline date.

Application Mediasite Presentation: A MediaSite presentation provides a tutorial for completing, saving, and submitting the SES online application. MediaSite features audio, video, and a PowerPoint presentation. To view the presentation, go to the catalog of DPI presentations at https://media2.wi.gov/DPI/Catalog/. On the Catalog screen, on the left side navigation bar, choose "Title I & School Supoprt." Then choose "SES Application Help" from the list. The presentation should begin automatically.

The annual deadline for applications is APRIL 1, 2008. Applicants will be notified of approval or denial via certified mail 4-8 weeks after the application deadline.
