Supplemental Educational Services (SES) Provider Application Information


The application for new providers to join the Supplemental Educational Services Program must be completed on-line. Detailed instructions are provided in the online environment. Each applicant will have to create a unique password to begin the application process. The application will require information about the type of services the organization intends to provide and documentation of the applicant's ability to provide an educational program that will meet the needs of eligible students and their families. Applicants must also 1) submit an Assurances and Financial Soundness survey, 2) submit a Certification of Debarment survey, and 3) provide evidence of financial soundness in hard copy. Complete applications include a submitted online application, both submitted surveys, and appropriate evidence of financial soundness.

Assurances and Certification of Debarment: SES provider applicants in Wisconsin are required to submit both the Assurances and Certification of Debarment surveys.

The deadline for applications is APRIL 2, 2012 at 3:00 PM CST. Applicants will be notified of approval or denial via certified mail 4-8 weeks after the application deadline.

Provider Fiscal Year:
A fiscal year is a period of time during which financial transactions are identified for administrative and accounting purposes. The Wisconsin school district fiscal year starts July 1st and ends June 30th. Some organizations use the calendar year, January 1st to December 31st, as their fiscal year for accounting and tax purposes. SES providers in Wisconsin will be asked to submit audits on a regular schedule determined by their identified fiscal year. Applicants may wish to consult with an accountant to determine which fiscal year would best fit their organization's functions and objectives.
