The templates below and on the following pages were created for LEA use in developing applications for funding under the Carl D. Perkins Career and Technical Education Improvement Act of 2006. If you have questions on the use of these templates please contact Denise Byrd at (608) 267-2274 or denise.byrd@dpi.wi.gov. These templates must be used when submitting an original or revised application to the Department of Public Instruction.
