The following templates were created for LEA use in developing applications for funding under the Carl D. Perkins Career and Technical Education Improvement Act of 2006. If you have questions on the use of these templates please contact Marilyn Bachim at (608) 267-2274 or

Instructions for Completing a Single District Application

Enrollment Chart (Section III-I)2008-09 enrollment in CTE programs and Career and Technical Student Organizations (CTSO)

CTSO Activity Chart (Section III-2) 2008-09 Activities that are integrated into the CTE curriculum.

Program of Study (POS) Chart (Section X, Part A) Complete one chart for each selected Program of Study.

Program of Study (POS) Description (Section X, Part B) Complete one chart for each selected Program of Study.

Program of Study Operational Plan (Section X, Part C) The operational plan provides a description of how the applicant plans to budget local and CPA money for all selected Programs of Study.

PI-1303-A (Rev. 04/08), Carl Perkins Basic Grant Application Budget/Budget Modification The form is used to develop the budget and must accompany the application (PI-1303). The form is also used to request a modification to the budget as needed throughout the grant period.
